Yup, exactly. There is a scene where he is confronted by a guard and the guard throws him into the DNA Mutagen storage machine and the mutagen and the Anabolic Plasma fall to his head and arms. That explains why he's bald at the end of the flick. There was gonna be a scene showing his hair falling off, but it was left on the cutting room floor.

Yes. It was based on the alternate red color scheme, but it was still different. However, once again, speaking physically, she was perfect. Everyone may have looked slightly different, but they got the physical looks right. Even Dee-Jay looked perfect despite wearing a Bison uniform most of the time:

Yes, there were some design changes, but they were quite minor. In fact, characters like Ryu and Cammy have some costume differences, but they both look perfect (Kylie looks exactly like Cammy). Honda too, despite being Hawaiian, he looked almost exactly like his video game counterpart.

Yeah, Zangief, Bison and Vega looked perfect.

Sagat is not as tall as his video game counterpart (he's a little bit shorter than Bison), but he looks perfect.
